What to do next

Ordered by exposure, and inside a band by what it costs to settle — of the things that could end this, the one you can find out about today is at the top.

  1. 1highB1 Hospitals, wound clinics, nursing homes and home-care agencies in the United States must pay more for each cleaning session with this system than for saline and a syringe. They pay from their own supply budgets, because no insurer pays for it separately, and the price has to cover what a session costs to make. If they will not pay more, nothing here earns money.
Precisely: US hospitals, outpatient wound clinics, skilled nursing facilities and home health agencies, buying out of a supply budget that sits inside a bundled payment and with no separate Medicare payment for the product, pay a per-session price for one single-use head plus its share of a per-patient bag that is above saline with a syringe or a bottle of HOCl cleanser, and that price covers the fully loaded cost of a session at the business case's volume and at one-tenth of it

  2. 2highB2 The single-use spray head has to fit only this maker's bag, and the bag's port has to seal itself between sessions. The United States regulator has to accept a connector that is not the standard one, and hospital buyers have to accept a head that will not fit the bags they already stock. If any of that fails, the head becomes an accessory for anyone's bag and the bag has to sell on its own, which is a different business.
Precisely: The single-use head is keyed to this maker's bag, and buyers accept it. One connector can be made that accepts only this maker's head, reseals between sessions, and is acceptable to FDA (US Food and Drug Administration) within a 510(k) (premarket notification) or exempt route. US value-analysis committees and group purchasing buyers put on formulary a head that fits only its own maker's bag, and do not require it to fit bags they already stock

  3. 3moderateB8 In Denmark and the rest of Europe, the bag should fall in the second-highest of the four device risk classes rather than the highest. Danish regional tenders should let the bag and the head bid separately and score more than price, and no European patent should block the product. None of this has been checked against a primary source, because the tools here reach only American records. If it fails, Denmark comes later or costs more, and the American plan is unchanged.
Precisely: The referrer's home market is open. Under EU MDR (medical device report) Annex VIII, an HOCl irrigation solution with HOCl as a preservative is Class IIb, not Class III (FDA device class III), with a notified-body precedent. Danish regional tenders, Region Nordjylland's being the one in the file, admit the bag on its own and a head on its own, and weigh at least one criterion other than price. 4. 4moderateB3 Once a bag of the hypochlorous-acid saline is opened, it is reconnected to a fresh head at every session. It has to stay clean and within its chemical specification for longer than the usual gap between one patient's sessions, and a company that is not a competitor has to be willing to fill it. If the opened bag cannot be kept, it cannot be labelled for repeated use. Under the United States regulator's proposed rule, the bag would then have no label it can carry.
Precisely: The opened per-patient bag holds, and someone will make it. HOCl-preserved saline in a bag that a new head re-enters at every session stays within its free available chlorine, pH and preservative-effectiveness specification. Wound organisms introduced at the port do not survive in it. Both hold on three lots aged to the end of shelf life, for an in-use period at least as long as the usual gap between sessions in every setting sold to. A filler that does not itself sell HOCl wound cleansers in the US will make the bag with a keyed port

  5. 5moderateB4 Squeezed by hand, the head has to deliver the pressure range that wound-care guidance recommends, every time and for every nurse. It has to clean at least as well as a syringe, last a full session, and not put chlorine into the air the nurse breathes. If it cannot do this by hand, it needs a pump or a redesign, which changes the device and its route to market.
Precisely: The hand-squeezed head works and is safe to use. Fed from the bag through its check valve, it holds 4-15 psi at its working distance for every operator, from the first to the last 10% of the bag. It removes slough and debris no worse than a 35 mL syringe with a 19G angiocatheter. After sterilization and ageing it survives twice a full session's squeeze cycles. 6. 6moderateB6 Several patents on splash shields and wound-washing hardware are still in force, some until about 2040, and a competitor holds a patent family on stabilised hypochlorous acid. Nobody has yet checked any of them against a defined head, because the head has not been drawn. A patent lawyer has to find a clear path or a licence, and the person who referred the idea has to be able to bring the rights to the original design. If not, the head must be redesigned, licensed or delayed, or the bag sold for someone else's hardware.
Precisely: Nobody else's rights stop a US sale of the head and the bag at launch. Counsel finds that no in-force claim or design reads on a defined head geometry, or a licence or design-around exists. That covers the Schultz utility and design patents, Medaxis, the '811 and '457 leads and the pending CareFusion applications. No Urgo or Sonoma solution claim reads on the chosen formulation. 7. 7moderateB7 In the United States, the bag should clear by copying the hypochlorous-acid wound washes already on the market, and the head as an ordinary irrigation device, both without a clinical trial. Only the regulator can confirm the head, and asking means disclosing the project. If the head needs a new classification, or the regulator's pending rule turns against the bag, the timeline and the money needed both change, and so may who can carry the programme.
Precisely: Neither half needs more than a 510(k). FDA places the separately sold, hand-squeezed head in 21 CFR (Title 21 of the Code of Federal Regulations) 880.5475 or 880.6960 and does not require a De Novo (De Novo classification request). The bag stays a 510(k) wound wash under product code FRO with HOCl as a preservative, because proposed rule 2023-26209 is either not finalized against it or finalized with HOCl in its Class II (FDA device class II) low-AMR-concern tier. 8. 8lowB5 Nurses and wound doctors need a reason to pick this over saline and a syringe. The label cannot say that hypochlorous acid helps wounds heal, and no adequately sized trial showing that it does has been found. So the reason has to be their own belief, or a shield that measurably keeps splash off them. If neither holds, the system still works but sells on convenience alone, for less.
Precisely: Clinicians have a reason to choose it that the product can carry. The label may not say that HOCl acts in the wound. So either US wound clinicians already believe that HOCl cleansing beats saline, or the shield measurably cuts splatter on the irrigator with no detectable chlorine. And irrigating at room temperature instead of warmed costs nothing clinically

The assessment, as written the bet, each pillar's reasoning, the regrades

Assessment — idea-023

A wound irrigation system for US professional wound care. It pairs a per-patient bag of saline preserved with hypochlorous acid (HOCl) with a single-use, hand-squeezed three-nozzle spray head that has an integral splash shield and fits only this maker's bag. It is a variant of "Aquaceso", which a Danish contact of the owner referred in.

This file carries no confidence tags. It cites them: [claim N: tag] reads claim N of the candidate file with the tag it carries there, and check_assessment.py refuses a citation that disagrees. It moves no score, no stage: and no status:.

How these gradings were made. Read this first, because they are not prospective. This is the first assessment of idea-023. It was stated on 2026-10-04 against P1 per-patient HOCl bag, the shape recorded in the loop record as a redesign. Every consequence class below was set on 2026-10-04, after the Verifier had tagged claims 1-194. The only exception is claim 195unverified, which this file appended (pillar B2 The single-use head is keyed to this…) and which nobody has checked yet. So every grading here is a reconstruction, and the graded: dates cannot show otherwise. The damage was limited the same way the loop record limits it. Each class was set from what the register's "If it is false" column and the binding chairs' positions say breaks, not from how the tag came back. No claim under any pillar is refuted, so no grading here was chosen to keep a refutation away from a fatal pillar.

Register coverage made several rests on: lists wider than this file would have drawn them. The checker requires every claim cited in a live row of the loop record's assumption register to sit under some pillar. That is 116 claims, plus claim 195unverified. Each claim was placed where its own row's "If it is false" column lands. Where that pulled in a verified claim the pillar would survive without, and so lifted its standing, the last section says so. That applies to B1 US hospitals, B3 The opened per-patient bag holds, B4 The hand-squeezed head works and is safe to…, B6 Nobody else's rights stop a US sale of the… and B7 Neither half needs more than a 510.

What the referrer asked, and where each answer sits. The referrer asked about the clinical, regulatory, intellectual-property and market position. In this file the clinical position is mainly B5 Clinicians have a reason to choose it that…, with B4 The hand-squeezed head works and is safe to… for safety in use. Regulatory is B7 Neither half needs more than a 510, plus B3 The opened per-patient bag holds for the bag's label. Intellectual property is B6 Nobody else's rights stop a US sale of the…, with B2 The single-use head is keyed to this… for whether the keyed fit can be owned. The US market is B1 US hospitals and B2 The single-use head is keyed to this…, and Denmark and Europe are B8 The referrer's home market is open. Under…. Every European and Danish claim in the file is unconfirmed, because no connector here reaches a European source. B8 The referrer's home market is open. Under… says so rather than borrowing the US result.

The bet

A wound nurse cleans a chronic wound at each dressing change. She may work in a US hospital, an outpatient wound clinic, a skilled nursing facility, or on a home health visit. Today she uses saline and a syringe, or pours from a bottle or bag of an HOCl cleanser that is already cleared claim 6verified. The bet is that her employer will buy a system instead. It has two parts:

  • The bag. A sterile bag of saline preserved with HOCl, opened once per patient and used over a labelled in-use period. That is how the one cleared HOCl irrigation bag is labelled claim 122verified.
  • The head. A fresh single-use spray head at every session. The nurse squeezes it to reach the wound-cleansing pressure range, and its shield keeps the spray off her.

Nobody pays for the system separately:

So the facility or agency pays out of its own supply budget. The price has to beat the cost of saline and a syringe, or of an HOCl bottle, on cost-in-use claim 161unconfirmed.

The money is meant to come from the head: one per session, fitting only this maker's bag claim 162unconfirmed, claim 195unverified. The bag is a parity product. On the label, HOCl has one job, which is to preserve a bag that is opened more than once claim 119verified. The label makes no claim that HOCl does anything in the wound.

This shape came out of a redesign. The original single-use kit ran into two problems:

  • FDA's 2023 proposal accepts a preservative only in a nonsterile wash or a multiple-use wash for one patient claim 119verified.
  • A hand-squeezed bag peaks at about 4 psi claim 143verified.

Two things are unknown and sit under the whole bet: who makes Aquaceso, and whether the referrer can bring its rights claim 2unconfirmed, claim 3unconfirmed.

The shapes it is not. Each one survives a different pillar failing. None is chosen here. This /pursue run has spent its pivot budget, so moving to any of them is the owner's decision.

One shape question is open, and it is the owner's to answer. Neither intended_use: nor mechanism: says who holds an opened bag between outpatient clinic visits. There are three options: the clinic keeps it, the patient takes it home, or it is thrown away after each visit. Each option changes the label. B1 US hospitals's cost per session and B3 The opened per-patient bag holds's in-use period both turn on the choice. The loop record handed this to the owner (REQ-0235 For the P1 per-patient HOCl bag), and this file does not make it.

Pillars

There are eight. One is graded fatal: somebody has to pay more per session than for saline and a syringe, and enough to cover the cost (B1 US hospitals). Five are reshaping. Each of them names a neighbouring shape above, and that shape survives the pillar failing. Two are costly: the clinical reason to choose the system (B5 Clinicians have a reason to choose it that…), and the Danish and European route (B8 The referrer's home market is open. Under…). The last section argues against three of these gradings.

Exposure is not truth, and no band here is a verdict. critical means an unanswered question that could end the thing, which is the ordinary condition of an early opportunity. resolved means the claims under a pillar came back verified, not that the pillar is right. And these bands do not compare between ideas — they say where attention goes next on this one.
