Reference products and why people stop using them — veterinary diabetes glucose monitoring

1. Summary

The domain dossier's own reference-products section (§5) already carries a detailed abandonment record for serial blood-glucose-curve sampling and for off-label FreeStyle Libre use, built from six accuracy PMIDs and one owner-survey PMID. This report adds material the dossier does not yet contain, located in this pass: the portable blood-glucose meter used as the accuracy reference in nearly every study the dossier cites — Zoetis's AlphaTRAK line — has its own small, independent accuracy literature that the dossier does not currently name. Separately, one study varied the FreeStyle Libre's sensor placement site itself (hip versus the standard scapular position) in dogs, and three further peer-reviewed FreeStyle Libre cohorts exist beyond the six PMIDs the dossier already lists, one of which states outright that the device "cannot be recommended for routine monitoring" of a specific patient population. Commercial-timeline material for GluCurve Pet CGM and AlphaTRAK 3's own 2023 US launch was also located via web search but is not carried into this report's Details as a cited finding, because none of the trade-press or manufacturer press-release sources found for it carries a resolvable identifier in this fleet's closed citation vocabulary — that gap is disclosed in §5 rather than papered over with an uncitable source.

Sensor placement site — a product-use variable the dossier does not address

Every FreeStyle Libre accuracy study the domain dossier cites uses the standard scapular sensor placement site. One study varied this directly: in 24 non-diabetic dogs undergoing insulin-induced hypoglycemia, FreeStyle Libre 14-day sensors placed simultaneously between the scapulae and over the hip showed a constant bias of 5.6 mg/dL between the two sites (95% limits of agreement -26.3 to 37.5 mg/dL) and correlated with each other (r=0.731, P<.001), measured against blood glucose by AlphaTRAK 2 [1]. The hip site met clinical- but not analytical-accuracy criteria, with 100% of paired hip-site samples falling in Parkes error grid zones A (83%) or B (17%) [1]. The authors' own stated conclusion is a qualified one: hip placement "may be an acceptable alternative to placement between the scapulae," but "the utility in hyperglycemic dogs is unknown" [1] — the study was conducted only in induced hypoglycemia, not spontaneous diabetic hyperglycemia [single-source]. This bears on the dossier's §5 abandonment record from the product-flexibility side rather than the failure side: it does not establish whether an alternate site reduces the skin-reaction or detachment rates the dossier already documents at the scapular site, but it is the only evidence found that the sensor has a viable alternative attachment location at all.

AlphaTRAK — accuracy of the reference meter itself, not just its use as a comparator

The domain dossier's accuracy table uses AlphaTRAK 2 as the blood-glucose comparator in every FreeStyle Libre study it cites, but does not independently source AlphaTRAK's own accuracy. In a prospective study of 60 client-owned cats, a beta prototype of the newer AlphaTRAK 3 meter met ISO 15197:2013 accuracy criteria in 96% of measurements and fell within consensus error grid zones A and B in 100% of measurements against a reference laboratory standard, with a correlation coefficient of 0.99 across the device's stated 20-750 mg/dL range and no significant constant or proportional bias on Bland-Altman analysis [2]. This is the only independently retrieved ISO-accuracy validation of the current-generation AlphaTRAK device found in this pass [single-source]; no comparable independent validation of AlphaTRAK 2 itself — as opposed to its routine use as the comparator device throughout the FreeStyle Libre literature — was found.

Additional FreeStyle Libre cohorts extending the abandonment record

Three further peer-reviewed cohorts, none among the six PMIDs the dossier's §4/§5 already cite, extend its FreeStyle Libre accuracy and tolerability record. In 41 client-owned diabetic cats, sensor placement was well tolerated (70% of cats showed no reaction) and owners were reported as "very satisfied," but only 53% of interstitial readings fell within 15% of the paired AlphaTRAK value, ISO 15197:2013 criteria were not fully met, and median sensor functional life was 10 days (range 1-14) [3] [single-source]. In 7 dogs with diabetic ketoacidosis, FreeStyle Libre readings correlated well with AlphaTRAK 2 (r=0.89), but the mean absolute relative difference was 25.2% (range -70.4% to 101.9%), ISO criteria were not met, and accuracy was measurably worse specifically in the hypoglycemic range [4] [single-source]. Most directly bearing on abandonment: in 10 client-owned cats with diabetic ketosis or ketoacidosis, 81% of high-glycemic-range FreeStyle Libre 2 readings deviated more than 15% from the paired portable-meter value, and the study's own authors concluded, in their own words, "currently this method cannot be recommended for routine monitoring of cats with DK(A)" [5] [single-source] — a stated clinical recommendation against routine use in this specific population, not a limitation left for a reader to infer. A contemporaneous review of the same device across both species states the pattern plainly: FreeStyle Libre "has adequate clinical accuracy both in dogs and cats, even though the accuracy is lower in the hypoglycemic range" [6] — a synthesis consistent with, but not independent evidence beyond, the primary cohorts above [single-source].

Thin: Every substantive line in §3 rests on exactly one cohort or case series and carries [single-source] — the hip-placement finding (n=24 dogs, induced hypoglycemia only), the AlphaTRAK 3 accuracy finding (n=60 cats, beta prototype device), and each of the three additional FreeStyle Libre cohorts (n=41 cats, n=7 dogs, n=10 cats respectively).

No independent ISO-accuracy validation of AlphaTRAK 2 itself (as opposed to its role as a comparator device) was found; one targeted query was run and returned zero PubMed results, so a broader phrasing may still surface one and this is recorded as not-yet-found rather than a settled negative.

[inference] Whether a hip-site FreeStyle Libre placement would reduce the skin-reaction or detachment rates the domain dossier's §5 documents for the standard scapular site is not tested by [1] or by anything else retrieved in this pass — the hip-site study measured only agreement between sites during induced hypoglycemia, not complication rates.
