1. Summary

The domain dossier's §3 already lays out the five-role table from its one owner-practice survey (Cooley et al., PMID 28948902) and names three open gaps: no source separates veterinary-technician incentives from veterinarian incentives, no source addresses clinic-side economics of supplying fluids, and no source measures the purchaser role in dollar terms. This report does not close those gaps outright — no device-specific or fluid-therapy-specific literature for any of them was found — but it adds two feline-CKD-specific surveys not previously mined for user-group content (an owner caregiver-burden survey and a practitioner-management survey, both distinct from the dossier's Cooley source), plus general small-animal-practice economics literature on financial-limitation distress, technician turnover costs, online price opacity, and insurance-driven spending behavior. 3. Details

Prescriber — guideline awareness is high, but owner constraints already limit what gets done

A 2023 questionnaire study of 409 veterinary practitioners in Portugal found that although 92.7% (379/409) reported using IRIS guidelines to manage feline CKD, only 35.7% (146/409) were actually able to comply with the guideline-recommended 2-3-month (or more frequent) recheck interval for stable patients, "due to owners' constraints" — the study's own stated reason [single-source] [1]. The same study found systolic blood pressure was measured systematically in only 19.1% (78/409) of patients despite near-universal guideline awareness [single-source] [1]. This is a direct, feline-CKD-specific instance of the pattern the dossier's §3 already flags qualitatively (a guideline-endorsed intervention constrained by something other than the guideline itself) but is drawn from a different, non-US population (Portugal) and is not evidence about the specific subcutaneous-fluid-therapy recommendation.

A 2017 JAVMA cross-sectional survey of 1,122 US and Canadian small-animal practitioners (37,036 surveyed) found 57% (620/1,088) reported client economic limitations affected their ability to provide desired care on a daily basis, and 49% (527/1,071) reported moderate-to-substantial burnout, with economic limitations cited as a contributing factor [single-source] [2]. This is general small-animal-practice economics, not CKD- or fluid-therapy-specific, offered as the closest US-based measure of how a prescriber's recommendation environment is already shaped by a client's ability to pay.

Operator — the veterinary technician carries training load and distress the dossier's gap names but cannot yet quantify for this domain

The dossier's §2 records that in-clinic owner training for home fluid administration is typically performed by "veterinarian/technician." A 2025 JAVMA survey of 207 emergency veterinarians and 154 emergency veterinary technicians in the US found nearly 95% of veterinarian respondents reported seeing owner-reported financial limitations affecting patient care at least weekly, and reported that technicians expressed distress about being unable to provide recommended care more often than veterinarians did [single-source] [3]. This is emergency-practice-specific, not chronic-disease-management-specific, but it is the only source located this pass that separates a technician's experience from a veterinarian's at all — a partial, directional answer to the dossier's own named gap, not a resolution of it. A separate 2026 JAVMA paper states plainly that "workforce shortages and turnover among credentialed veterinary technicians impose substantial operational and financial costs on veterinary practices" and that "veterinary technician-specific transition-to-practice research remains limited" [single-source] [4], which bears on whether the technician workforce that currently performs owner training for home fluid administration is a stable one; no quantified turnover rate specific to feline-CKD or fluid-therapy training was located.

Purchaser — a price-opaque channel, not fluid-therapy-specific

A 2026 cross-sectional study of 177 US small-animal veterinary clinics (157 with a website) found only 3 (1.91%) published specific service pricing online, only 7 (4.46%) offered a "Wellness Plan," and only 2 (1.27%) mentioned insurance options on their sites [single-source] [5]. This does not report fluid-supply or implant-kit pricing specifically — it is general small-animal pricing opacity — but it bears directly on the dossier's own named gap that no source measures the purchaser role in dollar terms: an owner in this domain has, per this study, a low probability of finding a price for anything before a treatment decision is made, whatever the item.

Payer — still functionally absent; where insurance exists, it changes spending

Consistent with the dossier's own §6 finding that no federal or near-universal private payer for veterinary devices exists, the only payer-adjacent evidence located this pass concerns private pet insurance uptake and its effect on spending, not coverage of subcutaneous fluid therapy specifically. A 2021 US choice-experiment survey of 306 uninsured dog owners found insurance uptake increased 12.3% after treatment-cost and disease-risk information was presented, and 44% of respondents were willing to spend up to $1,000 and 46% were willing to spend $3,000 or more to prolong their pet's life by a year [single-source] [6]. A 2020 US logit-model study of dog owners found pet health insurance "had a significant and positive impact on the amount spent at the veterinarian" [single-source] [7]. Both studies surveyed dog owners, not cat owners, and neither addresses CKD or fluid therapy; they are offered as the closest available evidence that insurance status, where present, changes spending behavior in the same US companion-animal market this domain sits in.

Patient (cat) and the person acting on its behalf — a feline-CKD-specific caregiver-burden survey

A 2025 survey of 100 UK owners of cats with CKD found 99% reported feelings of anxiety associated with caring for their cat, 66% said their daily routine had changed a lot since diagnosis, and 37% said caring for their cat negatively impacted or restricted their own life [single-source] [8]. The same survey found extreme worry, anxiety or concern related to prognosis was reported by 72% of owners at diagnosis and 50% at survey completion, and the authors described owners as "reported experiencing feelings of anxiety associated with caring for their cat, suggesting caregiver burden," calling for "veterinary professionals, including veterinary nurses" to play a greater role in education and support to alleviate it [8]. This survey did not ask specifically about subcutaneous fluid administration as distinct from CKD management generally, and its sample is UK, not US; it is the single most direct piece of published evidence in this domain that the "operator" role (whether owner or technician) carries a burden the guideline-content sections of the dossier do not capture.

The structural fact that no federal or near-universal private veterinary payer exists is not re-derived here; it is the dossier's own §6 finding (sourced there to 21 CFR 807.65(b)) and is only referenced by this report, not re-established.

Thin: All eight sources are [single-source] findings, none upgraded to plain fact regardless of how authoritative any one source is.
