Research report

A research report is the sourced material a domain dossier is synthesized from — generated on a plan and a cadence, one topic per file. A report carries no confidence tags. Its bracketed markers say who might have an incentive to shade a line; none of them says anyone checked it. To reach a score, a line has to be drafted onto a candidate as unverified and pass the Verifier or the Corroborator, like everything else.

Domain: ctdna-mrd-assay-validation · Scope: Publicly readable primary text only, per REQ-0028 scope note. 42 CFR 493.1253/.1255/.1256 via the eCFR connector; CAP's own public checklist excerpts and change summaries plus CAP-published CAP TODAY articles; standards-body public abstracts for ISO (International Organization for Standardization) 17034:2016 and CLSI EP17-A2; AMP/CAP NGS (next-generation sequencing) and cfDNA (cell-free DNA) validation guidelines at whatever depth was openly reachable. Excludes the ISO 17034:2016 full text (purchase), the current CAP Molecular Pathology (MOL) checklist (restricted to CAP-accredited laboratories), CLSI EP17-A2 full text (purchase); no paywall was crossed. US jurisdiction only.

Sourcing: Rests on CLIA (Clinical Laboratory Improvement Amendments) regulatory text (three eCFR sections) and CAP's own public 2017 All Common checklist excerpt for what is required, and on the AMP/CAP 2017 NGS guideline full text for what is recommended; the current MOL checklist wording, ISO 17034 body text and the 2023 AMP/CAP cfDNA guideline full text were not read.

What ISO 17034, CAP MOL checklist and CLIA 493.1253 require, accept, or are silent on for third-party reference material in LoD validation

1. Summary

For a laboratory-developed test, both CLIA and CAP's All Common checklist require the laboratory itself to establish analytical sensitivity (lower detection limit) before reporting patient results [1][7]. In the text of both that was read, the analytical-sensitivity requirement names no material type, no third-party source and no traceability condition [1][7]. The AMP/CAP 2017 NGS oncology-panel guideline is the only document read that recommends reference materials by name, in "should" language [4]. The current CAP MOL checklist and the ISO 17034 body text were not readable without crossing a paywall, and neither was the full text of the 2023 AMP/CAP cfDNA guideline [5][8][11].

2. What Changed

_Baseline (v1). No prior version; this establishes the starting point for future diffs._

3. Details

CLIA (42 CFR Part 493) — requires LoD; silent on the material

CAP checklist — requires LoD; accepts reference materials as one option

ISO 17034:2016 — addressed to producers, not to laboratories

Professional guidelines — recommend (2017 NGS); unknown (2023 cfDNA)

5. Sourcing & Gaps

Well established: A laboratory must establish analytical sensitivity for an LDT (laboratory-developed test) before clinical use: CLIA 493.1253(b)(2)(iii) [1] and CAP COM.40400 [7] are separate documents from different bodies. The same pair is also silent on the material used for that establishment.

Thin: Every other line rests on one source and carries [single-source]. The MOL-specific material options rest on one secondary CAP TODAY description of a 2017-era checklist [9]. The 2023 cfDNA guideline line is [paywalled]: only the abstract was read. Jennings [4] was read as full text through a fetch tool's extraction layer, not by direct text search. Two extraction passes disagreed on the context of the "59 samples" figure, so no LoD sample-size figure from [4] is reported here.

Rescoped from class 3: REQ-0028's "why" asks whether the asset in claim 19 is something "a fast follower cannot skip". That is partly a question about competitor behaviour. It was not answered. What was researched instead is the documentary twin: what each text requires, recommends, accepts or leaves unsaid.

Out of scope: ISO 17034:2016 clause text (the iso.org OBP preview and the ANSI preview PDF both returned HTTP 403, and the full standard is sold). The current CAP MOL checklist; per [8], it downloads only for users "from a CAP-accredited laboratory". CLSI EP17-A2 full text (sold). ISO 15189 and ISO Guide 35 were not in the manifest row.

Not searched vs. not found: Not retrieved, because of a block rather than an absence: the 2023 cfDNA guideline full text (jmdjournal.org and sciencedirect.com both returned 403; there is no PMC copy), the ISO catalogue page iso.org/standard/29357, and the Springer article on ISO 17034 (DOI 10.1007/s00769-017-1285-5, which redirects to a login). A Guideline Central summary of the 2023 recommendations was fetched but not cited, because it is a secondary summary. Current MOL.36118 text was searched for and not found in any public CAP document. MOL.31015 and MOL.31025, named in the scope note, were searched for and not found publicly. Their subjects are not established here.

[inference] ISO 17034's stated scope is producer competence [11]. On that reading it would bind a reference-material vendor and not the laboratory running the LoD study. A laboratory would meet it only indirectly, by choosing an accredited producer. The clause text was not read, so this reading rests on the public description alone.

[inference] Across the texts read, third-party traceable reference material for a laboratory's own LoD is required by none. It is recommended by one professional guideline [4], and one CAP accuracy requirement accepts it as one option among several [7]. On that basis, claim 19's characterization-dataset asset looks like a convenience, not a regulatory prerequisite. The unread current MOL text could change this.
