idea-003 · payer messages

Every line ends in the claim that substantiates it and that claim's own confidence tag, or is marked unsubstantiated. A message set never upgrades a tag, invents a number, or states anything the candidate's intended use does not cover.

Commutable patient-matrix reference material panel for validating tumor-informed ctDNA MRD assays (clinical QC material) — payer messages

Intended use: Quality control material — intended for use by molecular laboratories to monitor the analytical performance (analytical sensitivity at defined low tumor fractions, specificity, and inter-run precision) of tumor-informed circulating tumor DNA minimal residual disease assays, during assay validation and in routine run-by-run quality control. Not a calibrator, and not for use in establishing or interpreting an individual patient result.

Messages

Beyond intended use

Any message above that asserts a population, setting, duration or outcome the intended use does not state. None. is a valid answer and the honest one for most candidates. An absent section is a failure.

None. This is the set where the temptation to exceed the indication is strongest — a payer audience invites a patient-outcome or cost-offset story, and the customer's test has one to borrow. Checked and refused:

  1. No outcome is claimed. No message says the material improves MRD test accuracy for patients, changes a recurrence call, or reduces downstream spend. The intended use is analytical performance monitoring and excludes individual patient results. A line saying so to the payer directly ("no change in any patient's result, treatment or outcome, and no saving") was drafted and deliberately kept out of ## Messages: it is a restatement of the indication rather than a proposition any claim substantiates, and tagging it to an unrelated verified claim would have padded the verified count. Whoever says this set out loud should still say it.
  2. "Why a payer might still care" is conditional and analytical. It speaks only to whether a coverage process for the customer's test requires known-VAF (variant allele frequency) material for limit-of-detection work — which is assay validation, inside the indication — and it says the text has not been read. It must not be rewritten as "payers require this product".
  3. The customer's money is never presented as the product's. The 0340U rate, volume and MolDX listings appear as the payment this product sits inside, never as its price, market or coverage.

What we cannot say yet

Every message above whose backing is not verified. The checker enforces set equality — exactly these lines, no more and no fewer.

Healthcare Common Procedure Coding System — the US billing codes for equipment, supplies and some services; a code is how a payer recognises what to pay for

a Medicare local coverage determination number — the public id of one local coverage rule; the letter L and five digits

local coverage determination — a Medicare contractor's rule, valid in its region, saying when something is covered
MRD
molecular residual disease — cancer left behind after treatment, too little to see on a scan, detected through DNA in the blood
VAF
variant allele frequency — what share of the DNA fragments at one position carry the tumour's mutation; the lower it is, the harder the test
