Implanted feline subcutaneous fluid port — the story so far

How to read this. This is a synthesis, not a new source of truth: every claim it cites carries the tag the candidate file gives it right now, copied not paraphrased, and every line quoted from a deck is quoted verbatim. See docs/narrative-spec.md.


The one-paragraph version

This is an implanted, fenestrated subcutaneous port with a low-fouling surface and a metered owner-operated giving set, labelled for veterinary use only, aimed at cats with chronic kidney disease (CKD) whose owners currently give repeated needle-stick subcutaneous fluids at home. Regulatory burden is genuinely light — a manufacturer of devices used solely for veterinary purposes need not register or list with FDA [claim 2: verified] — but whether animal-only devices sit outside premarket review at all is a different, unresolved question [claim 1: unconfirmed], and the human-use analog this candidate deliberately avoids (implanted vascular access ports) is a real, crowded Class II field [claim 36: verified]. Demand is the best-evidenced part of the file: a survey found 85% of CKD-cat owners already giving home fluids, and 39% of those skipping doses because the cat was frustrated [claim 29: unconfirmed] [claim 30: unconfirmed]. Since the previous version of this narrative, a business case was added to the candidate file (2026-09-03), laying out nine further commercial assumptions — which prevalence figure to size against, retail price affordability, insurance offset, a compressed capital-and-calendar path to first sale, distribution, consumable margin, and a possible licensing route to an incumbent — every one of them freshly recorded as unverified rather than derived from anything already checked (claims 40-48 in the file). Everything downstream of the file's two verified regulatory facts — the oncologic hazard the design is built to reduce, reimbursement, TAM, freedom to operate, and now the entire commercial plan the business case lays out — remains open, and of 48 claims now in the file, only two have ever been verified.


The story so far

The problem this candidate targets is well documented in the one strong primary survey behind it: of 468 owners of CKD cats, 399 (85%) gave subcutaneous fluids at home, 57 (12%) did not, and 12 (3%) tried but could not continue [claim 29: unconfirmed]. Among the 399 actively administering, 155 (39%) reported skipping fluid administration in the prior 2-4 weeks because their cat was frustrated [claim 30: unconfirmed], and among the 57 who did not administer, 5 (9%) cited intolerance and 2 (4%) cited their own discomfort with the procedure [claim 31: unconfirmed]. That survey is US-majority but not US-exclusive — 75% of respondents were US-based, with the UK, Australia and Canada making up most of the remainder [dossier: feline-ckd-subcutaneous-fluid-therapy]. The candidate's premise — that the needle regimen lapses often enough to justify an implant — is therefore measured rather than assumed, even though the claim carrying that measurement is itself unconfirmed: a literature scan locates a paper and cannot appraise it.

The mechanism is an implanted, fenestrated diffuser at a defined, palpable, non-interscapular site, accessed by a metered giving set instead of a needle and hanging bag. The site choice is deliberate design rationale rather than an incidental detail: the one case report retrieved describes a soft tissue sarcoma developing at the interscapular site of a subcutaneous fluid port in a cat managed for roughly five years [claim 4: unconfirmed] — precisely the traditional site this candidate's intended_use: excludes — and feline injection-site sarcoma is asserted, but not itself verified, as a recognised entity associated with chronic inflammation at repeatedly-accessed sites [claim 5: unconfirmed]. The premise that a low-fouling surface and a better site actually reduce that risk relative to existing ports is the mechanism claim the Generator explicitly declines to assert on its own, and it rests on those two unconfirmed claims [claim 13: unconfirmed]. The nearest analog for how an implanted feline port behaves over years is the subcutaneous ureteral bypass (SUB) device, and its long-term record is genuinely two-sided: in 95 devices across 66 cats, chronic device mineralization occurred in 19-50% of devices and chronic urinary tract infection in 3-33% of cats, depending on flush protocol [claim 32: unconfirmed], while a generation-over-generation comparison in 80 cats found catheter kinking falling from 15.5% to 0% and 90-day survival rising from 75% to 94% [claim 33: unconfirmed] — evidence that implanted feline ports have characterisable, improvable failure modes, and that the incumbent device maker is actively improving them. The candidate's own claim asserting that the SUB device is fully established without qualification also remains unconfirmed [claim 6: unconfirmed]. Human implanted vascular access ports are offered as a second, more mature analog for infection risk alone — a prospective cohort of 249 ports recorded 0.05 catheter-related bloodstream infections per 1,000 catheter-days [claim 34: unconfirmed], and a US multicenter cohort of 1,714 ports recorded early infection in 1.2% with 0.2% port-related 30-day mortality [claim 35: unconfirmed] — but those are clinician-accessed ports in a different species and a different comorbidity profile, not the owner-accessed feline device this candidate proposes.

The regulatory shape is where the file's only verified ground sits, and it is narrower than it first appears. 21 CFR 807.65(b) verifiably exempts a manufacturer of devices used solely for veterinary purposes from establishment registration [claim 2: verified], and the listing half follows only by inference from adjacent scope language rather than an explicit carve-out. What is not verified — and what the whole "approximately zero review burden" framing depends on — is whether animal-only devices sit outside premarket notification and approval at all; that position lives in FDA Compliance Policy Guide territory no connector in this repo reaches [claim 1: unconfirmed]. The human-use analog this candidate is designed to stay clear of is now precisely identified rather than assumed: implanted vascular access ports are Class II under 21 CFR 880.5965, product code PXK [claim 36: verified], though no 510(k) clearances or recalls are recorded under that code in a 25-year openFDA window — a structural null about how the code is used, not evidence the human-use field is unregulated or trouble-free [claim 37: unconfirmed]. Reimbursement is a closed structural fact rather than a pending lookup: there is no third-party payer code of any kind for veterinary hydration therapy, and no CMS-backed connector was run because a null from one would be a category error, not a finding [claim 7: unconfirmed]. TAM rests on three Generator assumptions — population, price, and obtainable share — none reachable by any connector in this repo [claim 8: unconfirmed] [claim 9: unconfirmed] [claim 10: unconfirmed], and the two prevalence figures the research lane retrieved do not reconcile: 50% (and 68.8% in a DJD-recruited subgroup) in a US feline-only practice cohort against 1.2% in a UK primary-care cohort of 353,448 cats — the latter marked jurisdiction: US in the candidate file despite its UK source, a mismatch the Verifier flagged rather than resolved [claim 38: unconfirmed] [claim 39: unconfirmed]. Freedom to operate was never checked at all — the patent connector is unusable in this environment for want of a credential — against an incumbent (the SUB device manufacturer) shown to be an active, iterating device developer in this exact species and anatomy [claim 28: unconfirmed] [claim 15: unconfirmed].

A business case was added to the candidate file on 2026-09-03, after the Verifier's last pass and after the three decks below were built. It does not check anything the earlier passes left open; it converts the open commercial questions into nine explicit, individually-recorded assumptions, each tagged unverified on the grounds that it was "never checked; recorded so the Verifier can see it." It resolves the TAM ambiguity the way a usable business case has to, by choosing a side rather than settling the question: it adopts the US-practice cohort's 50%/68.8% prevalence figures as the more appropriate basis for sizing this candidate's addressable population than the UK VetCompass 1.2% figure [claim 40: unverified]. On top of that it assumes a retail price to the owner is affordable enough at the point of an elective decision that it does not itself suppress demand [claim 41: unverified], and that pet insurance offsets enough of the total owner cost for insured owners that the decision is not effectively priced out for that segment [claim 42: unverified]. It assumes first commercial sale is reachable on a calendar materially shorter than the combined capital-intensity estimate, because tooling, material qualification and sterile packaging validation do not require the multi-year case series to finish first [claim 43: unverified], and that the capital needed to reach that first sale — tooling, material qualification, sterile packaging validation alone — sits materially below the low-single-digit-millions figure given for the combined programme [claim 44: unverified]. It assumes a veterinary distributor or a direct-to-practice sales model can carry the device into feline-focused and referral practices at a cost and calendar the launch budget can absorb [claim 45: unverified], and that the manufacturing and distribution cost of the giving-set consumable leaves a viable recurring margin at the assumed $15-30/month price [claim 46: unverified]. It names, for the first time in the file, an alternative commercial path: that an existing marketed feline subcutaneous port manufacturer or the SUB device manufacturer would be willing to license or OEM-integrate this candidate's low-fouling surface chemistry and metered giving set into their own already-marketed product [claim 47: unverified]. And it assumes the teaching-hospital case series the mechanism claim depends on can itself be resourced and IACUC-approved on a calendar and budget shorter than the years implied by the combined capital-intensity estimate [claim 48: unverified]. None of these nine claims has been checked by any connector, deck, or Verifier pass since they were written, and the candidate file's own Verified Composite section — last recomputed 2026-08-29 against 39 claims — has not been recomputed to reflect them; the Claims section itself now holds 48.

Where the three lenses agree — and where they don't

All three decks pass check_decks.py as current — source_updated in each deck's frontmatter matches the candidate's updated: field of 2026-09-03, and no claim tag any deck cites has moved. But that currency comes from the mechanical restamp check_decks.py --restamp performs when a candidate edit leaves every existing tag untouched, not from the decks having been rebuilt since the business case was added. Each deck's own text was written 2026-08-30, against the candidate as it stood before the nine business-case assumptions above existed — none of the three decks quotes, discusses, or references claims 40-48. What follows is read against that content, dated 2026-08-30.

The desirability deck turns on whether an owner will actually consent to the intervention: the burden the device targets is measured (39% of active administrators skip doses from cat frustration [claim 30: unconfirmed]) and a precedent for feline owners consenting to an implanted port under anaesthesia exists in the SUB literature [claim 32: unconfirmed] [claim 33: unconfirmed] — but that precedent is for a rescue indication, not an elective one.

The viability deck turns on the calendar the capital estimate does not carry. The SUB device's own long-term record shows a fouling problem (19-50% device mineralization [claim 32: unconfirmed]) that would surface years before any oncologic finding could, and the file prices one case series against one endpoint rather than against both — a tension the business case's own compressed-calendar assumption [claim 43: unverified] does not resolve, since it assumes the case series can be decoupled from first sale rather than showing that it can.

The feasibility deck turns on the same oncologic premise the mechanism claim declines to assert: whether the material and site choices actually work, in a cat, over years — the one condition among all three decks that a case series, not a scan or a connector, can answer.

The load-bearing question

All three decks agree the decisive open question is oncologic: whether the material choice and non-interscapular site actually reduce chronic inflammatory and sarcoma risk relative to the implanted ports already in feline use, a question no connector can answer and only a multi-year prospective case series can settle — on a calendar the file's own SUB-device evidence suggests may not even be the first clock to run out, since device fouling in that analog occurs in 19-50% of cases [claim 32: unconfirmed], years before any tumour finding could arrive. The business case added since the decks were built now assumes, without having checked, that capital and calendar to first commercial sale can be decoupled from that mechanism question entirely [claim 43: unverified] [claim 44: unverified], that owners and insurers will absorb the resulting price [claim 41: unverified] [claim 42: unverified], and that the case series itself can be resourced on a shorter timeline than the combined estimate implies [claim 48: unverified]. So the question this file now turns on is not only whether the device is safe over years, but whether a commercial plan built to reach revenue ahead of that safety answer is itself viable on terms that, as recorded, have never been checked?
