Single-use canister-free NPWT for canine and equine distal-limb wounds — the story so far

How to read this. This is a synthesis, not a new source of truth: every claim it cites carries the tag the candidate file gives it right now, copied not paraphrased, and every line quoted from a deck is quoted verbatim. See docs/narrative-spec.md.


The one-paragraph version

This is a single-use, canister-free negative pressure wound therapy (NPWT) device purpose-built for open, contaminated, or free-skin-grafted distal-limb wounds in dogs and horses — a battery-powered disposable pump holding roughly -80 mmHg through a superabsorbent dressing, paired with a fur-compatible adhesive drape and limb-conforming interface aimed at the one problem the human-labeled predicate (Smith & Nephew PICO) already fails at off-label: keeping a seal on fur. As a veterinary-only device it faces no FDA premarket review and no third-party payer of any kind, which makes its evidence gap the mirror image of the human-market candidates in this repo — regulatory and reimbursement questions are close to moot rather than the bottleneck, and almost everything genuinely unresolved is clinical evidence, engineering, and freedom-to-operate. Of the file's forty-five claims, four are verified and all four are regulatory or bibliographic-existence facts rather than commercial or clinical ones; thirty-five are unconfirmed, none is refuted, and six brand-new claims sit at unverified — a business case's assumptions about equine TAM additivity, revenue timing, a platform-royalty exit, per-treatment margin, a narrowed canine-only launch, and distributor reach, added 2026-09-03 and not yet attempted by a Verifier pass. The clinical literature the research lane has read at abstract depth is now genuinely mixed rather than uniformly discouraging, but every one of those findings stays unconfirmed, because a literature scan can locate a paper and never appraise one. The file is wrapped and queued-research at 2/9 verified factors against the 6/9 threshold — a "we don't know yet" outcome, not a kill.

The story so far

The device sits almost entirely outside FDA's premarket regime. 21 CFR 807.65(b) exempts a manufacturer of devices used solely for veterinary purposes from establishment registration, and the parallel listing duty attaches only to a device intended for human use — a genuine, primary-sourced reduction in this candidate's own regulatory burden [claim 2: verified]. The human-use counterfactual for this same hardware — what it would be if labeled for people — is Class II under product code OMP, 21 CFR 878.4780, but that finding is scoped explicitly to the human comparison and evidences nothing about this candidate's own animal-only pathway [claim 4: verified]. The premise the whole regulatory story rests on — that FDA CPG Sec. 655.100 exempts animal-only devices from premarket notification or approval outright — is still unconfirmed, not because anyone doubts it but because no connector in this repo reads Compliance Policy Guide text; the CFR sections that do resolve verify a narrower, different document than the one the claim names [claim 1: unconfirmed]. FDA's retained post-market misbranding and adulteration authority is asserted but never queried [claim 3: unconfirmed].

On the money side there is no third-party payer to check against and no code to look up [claim 11: unconfirmed], pet insurance is claimed at under 5% of owned dogs [claim 12: unconfirmed], and all three TAM components — a caseload of 30,000-80,000 qualifying cases [claim 13: unconfirmed], a per-unit price of $180-$320 [claim 14: unconfirmed], and an obtainable share of 20-30% [claim 15: unconfirmed] — are Generator estimates the market connector returns as structurally unverifiable, because the connectors that produce real numbers elsewhere in this pipeline are CMS and openFDA, and neither sees a veterinary market at all.

The device does not enter an empty field. Human single-use canister-free NPWT (Smith & Nephew PICO) is already being bought and used off-label by exactly this candidate's target customers, documented in published canine and equine work whose bibliographic existence is verified [claim 8: verified] [claim 10: verified] — though that tag establishes only that the records exist, not what they found. The candidate's own flagged assumption is that veterinary surgeons would choose a purpose-built unit over continuing to buy that off-label human device at a comparable price, with no connector for purchasing preference and no primary research behind it [claim 19: unconfirmed]. And because there is no premarket review to slow anyone down, fast-follower risk is structurally elevated — a deduction resting on claim 1, itself still unconfirmed [claim 17: unconfirmed].

The clinical picture has moved since the file's first pass, from thin-and-negative to genuinely mixed, and none of the movement touched a tag. Against the device: a pilot study of the exact form factor (PICO 1.6) in beagles reported control wounds better on every measure named, with benefits described as "remain[ing] unknown" [claim 9: unconfirmed]; a controlled 10-horse study at this candidate's exact anatomical site — the distal limb — found NPWT gave significantly less wound retraction than calcium alginate in noncontaminated wounds [claim 20: unconfirmed]; and a 2026 equine commentary reports isolated PICO-system use insufficient to stimulate healing in a small case series [claim 39: unconfirmed]. For the device: a retrospective matched-pairs comparison of 50 dogs found significantly fewer complications and less sepsis with NPWT than foam dressing [claim 34: unconfirmed], a human systematic review of 29 studies (n=5,614) found the canister-free format reduced infection, dehiscence, seroma and necrosis versus conventional dressings [claim 33: unconfirmed], and three uncontrolled veterinary case series report favourable graft-survival and healing outcomes [claim 35: unconfirmed] [claim 36: unconfirmed] [claim 38: unconfirmed]. Every one of these stays unconfirmed for a structural reason rather than an unspent check: the literature-detail connector returns a scan result that carries no confidence-tag vocabulary and has no code path to verified — a retrieved abstract is a lead to read, not a verification.

The engineering bet is that a fur-compatible adhesive drape and a limb-conforming interface can hold a seal where the human predicate's does not — and the published record already names the failure mode this design is meant to fix: the AJVR pilot reports that a complete seal required adhesive spray, that vacuum maintenance "lessened over time," and that self-limiting skin irritation "in all dogs" hampered attainment of negative pressure [claim 9: unconfirmed]. None of the candidate's own bench claims about that drape — that it can hold seal at target pressure for 72 hours [claim 28: unconfirmed], that the interface survives normal animal behaviour [claim 29: unconfirmed], or that adhesive contact stays below the irritation threshold [claim 30: unconfirmed] — is checkable by any connector in this repo, because the hardware does not yet exist. A recall analysis of the human product code adds one checkable-adjacent datum: 51% (19 of 37) of FDA recalls for canistered NPWT pumps name a canister, dressing-connector, or tubing defect as the stated reason [claim 37: unconfirmed] — evidence, drawn from the human database because the veterinary one does not exist, about the failure mode this canister-free architecture is meant to remove.

The one condition on this file that would be a clean, automatic kill is intellectual property, and it is entirely unresolved. The two foundational Wake Forest NPWT patents came back unconfirmed on a missing credential rather than a finding [claim 5: unconfirmed] [claim 6: unconfirmed], and the live risk that could actually block this product — the Smith & Nephew PICO canister-free family and dressing-interface chemistry — carries no patent number to query at all, so it is unsearched rather than merely unverified [claim 7: unconfirmed].

The domain dossier adds context the candidate's own claims don't yet carry as evidence. No US or any-country incidence figure exists for distal-limb wounds severe enough to warrant NPWT in either species — the same gap the candidate's own caseload claim already names, confirmed independently here [dossier: canine-equine-distal-limb-wounds]. The dossier also reads the equine care pathway as a structural mismatch this candidate would have to engineer around specifically: in the largest equine case series, NPWT was used as a days-to-weeks inpatient-adjunct therapy rather than a send-home device, with system resets roughly every 4.5 days — distinct from the canine population, where send-home use is already described in the literature without that caveat [dossier: canine-equine-distal-limb-wounds]. And on the incumbent's abandonment record, at least one US veterinary NPWT rental vendor markets dedicated paid support for "seals, leaks, and dressing construction" — a vendor-disclosed signal that these are recurring operational problems worth building a support line around, not merely a hypothetical concern [dossier: canine-equine-distal-limb-wounds].

Since the last narrative build, the candidate file itself moved: six new claims were appended on 2026-09-03, all carrying confidence: unverified and each sourced as a "business-case assumption ... never checked; recorded so the Verifier can see it" rather than as a connector result. unverified is a different thing from unconfirmed here and worth keeping separate — these six have not yet been through a Verifier pass at all, where the file's other thirty-five open claims were attempted and came back inconclusive or structurally uncheckable. The six name: that the equine segment materially enlarges the addressable revenue pool beyond the canine-only TAM estimate and that the two species' figures are commensurable enough to sum [claim 40: unverified]; that first commercial revenue is reachable within a calendar window commensurate with the file's low-single-digit-millions capital estimate [claim 41: unverified]; that an existing NPWT-platform maker or veterinary rental vendor would pay a royalty or transfer price for a validated drape and interface module rather than declining to integrate a third-party component [claim 42: unverified]; that a per-treatment rental or service fee would cover application/retrieval/reprocessing labor and still yield positive margin [claim 43: unverified]; that a launch narrowed to canine free-skin-graft cases only would need a materially smaller or shorter case series than the full-indication capital estimate assumes [claim 44: unverified]; and that the distributor relationships the capital-intensity claim assumes can be built within that same capital and calendar [claim 45: unverified]. None of these six has moved any score, composite, or gate reading — the Scores table, Verified Composite, and Gate Check sections in the candidate file all predate them and have not been re-run since.

Where the three lenses agree — and where they don't

All three decks are current by check_decks.py — every claim reference resolves and no tag has drifted, and the frontmatter's source_updated: 2026-09-03 now matches the candidate. But that stamp was reached by restamping, not by rebuilding: each deck's content was last generated 2026-08-30, before the six business-case claims (40-45) above were appended, and none of the three decks discusses the equine-TAM, revenue-timing, royalty, margin, launch-scope, or distribution assumptions those claims name. A reader treating "not STALE" as "addresses everything currently in the file" would be wrong on that one point; everything else the decks say still tracks the candidate's current tags exactly.

The desirability deck reads the seal problem from the user's chair — whether the person applying this device, not the device itself, can make it work in an ordinary practice — and the published record already speaks to it directly: the AJVR pilot required adhesive spray for a complete seal and reported self-limiting skin irritation hampering negative pressure "in all dogs" [claim 9: unconfirmed].

The load-bearing question

If a fur-compatible drape can be engineered to hold a seal reliably in an ordinary technician's hands — the single mechanical fact all three lenses converge on — does the clinical record in this exact species and site, currently split between a favourable 50-dog canine comparison and an unfavourable controlled equine study at the distal limb, support prescribing it; is there room to build it at all under a live canister-free patent family this repo has never been able to search; and do the six commercial assumptions the business case has just added — an additive equine TAM, a reachable revenue window, a platform-royalty exit, per-treatment margin, a narrower canine-only launch, and buildable distributor reach — survive contact with research that has not yet touched any of them?
